Given names imply a certain level of familiarity. I suppose that you could compare it to calling someone by a nickname in most other contexts. Everyone calls me 'Kakyoin' because there's a sense of formality to family names that I prefer. Only my family calls me 'Noriaki'. ['Family' here having a loose definition.]
In Jotaro's case, I don't think he cares that much. When I met him I was already addressing him informally and he never bothered to correct me, so I just kept doing that after things got a little less chaotic.
Did you completely overlook that all of us used each other's last names aside from Jotaro? Constantly? I know you tend to miss details like that, but I thought it was a little obvious.
